TRAP

Can SBRT delay progression or next-line therapy for limited progression on an ARPI?

Prospective single-arm phase II; 86 recruited, SBRT delivered to 81; ≤2 progressing sites.

Key result

Median time to next treatment or death was 27.0 months. The 6.4-month median PFS and 39% 12-month PFS are different measures, not treatment arms.

Current-practice context · reviewed 14 October 2026

TRAP is prospective single-arm phase II evidence that SBRT may delay progression or next-line therapy for selected men with oligoprogressive mCRPC continuing an androgen-receptor pathway inhibitor. Without a randomized comparator or established OS benefit, it should be used selectively after multidisciplinary review rather than as a universal standard. Consider the number/location of progressing lesions, systemic disease control, prior radiation, toxicity and the patient's treatment goals.
