SWENOTECA clinical stage I NSGCT management programme

Clinical question

Does vascular invasion identify patients with sufficiently high relapse risk to justify adjuvant BEP in stage I non-seminoma?

Trial context
Population
745 patients with clinical stage I non-seminomatous germ-cell tumours in a prospective SWENOTECA programme
Intervention
Risk-adapted surveillance vs one cycle adjuvant BEP
Key endpoint
Relapse rate on surveillance according to vascular invasion
Time horizon / follow-up
Median follow-up 4.7 years
Absolute Effect

What changes for patients?

SWENOTECA demonstrated why pathology matters in stage I NSGCT: among patients managed with surveillance, relapse was 41.7% with vascular invasion versus 13.2% without it. After one course of BEP, relapse was 3.2% in vascular-invasion-positive patients and 1.3% in vascular-invasion-negative patients. Because treatment allocation was not randomized, these absolute differences should not be converted into a causal NNT. NNT is therefore not assigned as a trial-level causal estimate.

NNT: Not applicable; non-randomized risk-adapted programme

Treatment burden / harm

One-cycle BEP toxicity

NNH: Not quantifiable from this nonrandomized programme; chemotherapy burden should be discussed against relapse risk

Prospective management programme; treatment allocation was not randomized

Clinical interpretation

SWENOTECA supports risk-adapted surveillance and one-cycle BEP for stage I NSGCT, with relapse risk strongly associated with vascular invasion.

Current-practice context · reviewed 14 October 2026

SWENOTECA supports risk-adapted counseling after orchiectomy for stage I NSGCT: surveillance relapse risk differs substantially by vascular invasion, and one-cycle BEP is associated with low relapse rates in the programme. Because allocation was not randomized, the apparent difference between surveillance and adjuvant BEP must not be converted into a causal NNT. Surveillance requires reliable follow-up; adjuvant BEP trades a lower relapse risk for chemotherapy exposure and potential late effects.
