Design, population and eligibility

Design

Phase III, open-label randomized trial

Sample size

5,101 patients randomized: 2,549 ribociclib plus NSAI and 2,552 NSAI alone. Stage II–III HR-positive/HER2-negative early breast cancer.

Eligibility

Anatomic stage IIA with specified high-risk features, stage IIB or stage III HR-positive/HER2-negative disease. Premenopausal women and men received goserelin. Patients with very low-risk stage I disease were not represented.

Follow-up / maturity

Follow-up and endpoint maturity are stated with the reported efficacy results; further long-term updates are noted where applicable.

Treatment and comparator

Ribociclib 400 mg orally daily, days 1–21 of each 28-day cycle for 36 months, plus letrozole 2.5 mg or anastrozole 1 mg daily for 5 years; goserelin 3.6 mg every 28 days for premenopausal women and men. Comparator NSAI alone with ovarian suppression when indicated.

Primary and secondary endpoints

Primary endpoint: iDFS by STEEP criteria. Secondary endpoints include distant DFS, recurrence-free survival and OS.

Key efficacy results

At median iDFS follow-up 44.2 months, HR for iDFS was 0.72 (95% CI 0.61–0.84). Four-year iDFS was 88.5% with ribociclib/NSAI versus 83.6% with NSAI alone (absolute difference 4.9 percentage points; 95% CI for difference 2.7–7.1). Three-year iDFS was 90.8% versus 88.1%.

OS remains immature: HR 0.83 (95% CI 0.64–1.07). The four-year analysis was exploratory after the protocol-specified primary iDFS analysis.

4-y iDFS · NSAI

Kaplan–Meier landmark rates from the exploratory 4-year analysis. OS is immature; the reported HR does not establish an OS benefit.

Absolute effect

At 4 years, iDFS improved by 4.9 percentage points (88.5% vs 83.6%); approximate landmark NNT 21 to prevent one iDFS event by 4 years. This is based on fixed-time Kaplan–Meier rates, not median survival. OS remains immature, so no OS NNT is reported.

Trial-specific toxicity and treatment burden

In the 4-year analysis, grade 3 and grade 4 treatment-emergent AEs were 58.2% and 5.5% with ribociclib/NSAI versus 17.9% and 1.6% with NSAI alone; grade 5 events were 0.4% and 0.2%. Neutropenia and transaminase elevations are important. FDA labeling reports dose interruptions in 73%, dose reductions in 23%, and discontinuation due to neutropenia in 1.1%. CBC, liver tests, ECG/QTc and electrolytes require monitoring.

Quality of life and patient-reported outcomes

Long-term QoL interpretation must account for three years of additional oral treatment and frequent dose interruptions/reductions. No QoL superiority claim is made from the iDFS result alone.

Subgroups and applicability

The iDFS benefit was consistent across prespecified clinical subgroups, including stage and nodal status. The population includes stage II patients with additional high-risk features and stage III; it is not a blanket indication for low-risk stage I disease.

Current-practice context · reviewed 3 November 2026

Ribociclib plus an aromatase inhibitor is an adjuvant option for appropriately selected stage II–III HR-positive/HER2-negative patients according to current approvals and local guidance. Compare eligibility and benefit with monarchE; do not infer class effect from PALLAS.

Limitations and critical interpretation

Open-label study; OS immature. Three years of ribociclib adds substantial monitoring and dose-modification burden. Four-year analysis is exploratory; long-term cure and late toxicity continue to mature.

Bottom line: NATALEE demonstrates a sustained 4-year iDFS advantage after three years of adjuvant ribociclib plus NSAI. It is a recurrence-reduction result, not yet a proven OS advantage.
