In high-risk, node-positive HR-positive/HER2-negative early breast cancer, does 2 years of adjuvant abemaciclib added to endocrine therapy improve long-term outcomes?
Evidence snapshot updated 23 October 2026. Source links are provided below; this is an evidence summary, not a substitute for current local guidelines or individual clinical judgement.
Population and treatment
5,637 patients with HR-positive, HER2-negative, node-positive high-risk early breast cancer.
Intervention and comparator: Abemaciclib 150 mg orally twice daily for 2 years plus endocrine therapy for at least 5 years versus endocrine therapy alone.
ClinicalTrials.gov: NCT03155997 ↗
Key results
At median follow-up 76.2 months, 7-year IDFS was 77.4% vs 70.9% (HR 0.734, 95% CI 0.657–0.820); 7-year DRFS 80.0% vs 74.9% (HR 0.746, 95% CI 0.662–0.840). Seven-year OS was 86.8% vs 85.0% (HR 0.842, 95% CI 0.722–0.981; P=.027).
7-year invasive disease-free survival · 7 years
Absolute-effect interpretation: Absolute 7-year IDFS difference 6.5 percentage points; approximate NNT 16 over 7 years based on the reported fixed-time rates. The 1.8-point OS difference is also fixed-time, but NNT should be interpreted cautiously because survival estimates are time-to-event estimates.
Toxicity and treatment burden
Diarrhoea, neutropenia, fatigue and venous thromboembolism are clinically relevant risks. Monitor blood counts, liver tests and symptoms; treatment interruptions/reductions are common considerations. The 7-year report found no new delayed-toxicity concern.
Interpretation and limitations
This is a high-risk node-positive population, not all HR-positive early breast cancer. Cohort eligibility and local label/guideline criteria should be checked before extrapolation. The 2025/2026 overall-survival update is important: earlier reports had not yet demonstrated significant OS benefit.
Use endpoint definitions, follow-up, analysis population and comparator exactly as reported. Do not infer NNT from a hazard ratio or median survival. Subgroup findings should not be treated as predictive unless the trial design and interaction testing support that interpretation.
