LUNAR

Does neoadjuvant 177Lu-PNT2002 before SBRT improve PFS versus SBRT alone?

Single-centre randomized phase II; 92 randomized, 87 evaluable; 1–5 PSMA-PET lesions. SBRT vs 2 cycles 177Lu-PNT2002 then SBRT.

Key result

Promising phase II signal for radioligand priming before MDT; single-centre design and follow-up should be considered.

Current-practice context · reviewed 14 October 2026

LUNAR is phase II evidence that two cycles of the investigational radioligand 177Lu-PNT2002 before SBRT improved PFS in selected PSMA-PET-defined oligorecurrent hormone-sensitive prostate cancer. This strategy is not yet interchangeable with established systemic therapy or standard MDT approaches; confirm eligibility, trial maturity, availability, marrow/renal risks and current approvals, and discuss it in a specialist MDT or clinical-trial setting.
