LITESPARK-012

In previously untreated advanced clear-cell RCC, do belzutifan or quavonlimab add benefit when combined with pembrolizumab and lenvatinib, compared with pembrolizumab plus lenvatinib?

Population
1,688 patients with previously untreated advanced clear-cell RCC.
Arm A
Belzutifan 120 mg orally daily + pembrolizumab 400 mg IV every 6 weeks + lenvatinib 20 mg orally daily.
Arm B / control
Quavonlimab/pembrolizumab co-formulation 25/400 mg IV every 6 weeks + lenvatinib 20 mg daily, versus pembrolizumab 400 mg IV every 6 weeks + lenvatinib 20 mg daily.

Randomized, open-label phase III study with dual primary endpoints: PFS by blinded independent central review and OS. Both investigational regimens were compared with pembrolizumab/lenvatinib.

Result reported 21 April 2026

Neither investigational combination met the co-primary endpoints of PFS and OS versus pembrolizumab plus lenvatinib at the prespecified interim analysis. The sponsor's announcement did not disclose hazard ratios or detailed arm-specific event estimates; this page therefore does not fabricate numerical effect sizes or an NNT.

What can be concluded: Adding belzutifan or the quavonlimab/pembrolizumab co-formulation to lenvatinib/pembrolizumab did not establish an improvement in the primary survival endpoints in this first-line phase III study. This is a negative primary-endpoint result, not evidence of equivalence or proof that no subgroup can benefit.

Primary endpoint status · interim analysis

PFS: Co-primary endpoint not met versus pembrolizumab + lenvatinib.

OS: Co-primary endpoint not met versus pembrolizumab + lenvatinib.

The public sponsor announcement did not disclose arm-specific hazard ratios or fixed-time event rates. This qualitative status panel intentionally shows no fabricated numerical effect or NNT.

Critical appraisal

The result illustrates why promising biological rationale and earlier-phase signals do not guarantee phase III success. The study compared two investigational intensification strategies against an active IO–TKI control, not against sunitinib. Detailed peer-reviewed arm-level efficacy and safety data should be added when available; the public sponsor update reported the endpoint outcome but did not provide hazard ratios.

Do not infer a precise absolute benefit or harm from the headline announcement. No NNT/NNH is calculated without time-specific event rates and sufficiently detailed data.

Current-practice context

As of 14 October 2026, neither investigational LITESPARK-012 triplet should be used as a new first-line standard on the basis of this study. Pembrolizumab plus lenvatinib remains the active control regimen; choose among established first-line RCC options using IMDC risk, comorbidities, toxicity preferences and current local guidelines.

Freshness note: targeted review includes the sponsor's 21 April 2026 results announcement; numerical full-paper results not available in that release.
