GOG-0218
Does concurrent and maintenance bevacizumab improve outcomes when added to carboplatin–paclitaxel in advanced ovarian cancer?
One underlying study; aliases and follow-up reports consolidated. Primary discipline: Medical.
1,873 patients with incompletely resected stage III or stage IV epithelial ovarian, fallopian-tube or primary peritoneal cancer.
Newly diagnosed advanced disease after cytoreductive surgery.
1,873 randomised/evaluable as reported.
Carboplatin AUC 6 plus paclitaxel 175 mg/m² for six cycles; bevacizumab 15 mg/kg every 3 weeks from cycle 2 through cycle 22.
Carboplatin–paclitaxel plus placebo; a third arm received bevacizumab during chemotherapy only.
Endpoints and results
Primary PFS; OS and toxicity secondary.
- Median PFS 14.1 vs 10.3 months for bevacizumab continued through maintenance vs control (HR 0.717, 95% CI 0.625–0.824; p<0.001).
- Original overall-population analysis did not establish significant OS benefit.
- Hypertension requiring treatment 22.9% vs 7.2%; gastrointestinal-wall disruption requiring intervention 2.6% vs 1.2%.

Median PFS
months (median, not fixed-time rate) · Primary analysis. Bars compare reported medians only and are not fixed-time survival probabilities.
Interpretation: No NNT from median PFS. Fixed-time PFS rates at a prespecified horizon would be required.
Toxicity, discontinuation and quality of life
Trial-specific harms: Hypertension and GI perforation/disruption are important; also monitor bleeding, thrombosis, wound healing and proteinuria.
Treatment burden/discontinuation: Maintenance requires prolonged infusions and monitoring; exact discontinuation rates require the full report.
Quality of life: No numerical QoL advantage claimed.
Subgroups, maturity and limitations
Subgroups: Potential variation by baseline risk/residual disease should not be asserted without formal interaction evidence.
Maturity: Long-term follow-up available; original overall-population analysis did not establish OS benefit.
Limitations: Predates current PARP maintenance and molecular stratification; PFS benefit is not OS superiority.
GOG-0218 showed a PFS benefit from bevacizumab continued into maintenance, without significant OS benefit in the original overall population.
Current-practice context must be checked against regional guidelines, patient fitness, biomarkers, alternatives and regulatory status before clinical application. Clinical freshness review and browser-rendered QA remain pending.
