In selected platinum-sensitive recurrent ovarian cancer, does secondary cytoreductive surgery before chemotherapy improve overall survival compared with chemotherapy alone?

Study design, population and eligibility

Design and sample

Phase III randomized; n=485.

Population

Platinum-sensitive recurrence selected for potential complete gross resection; most received platinum chemo and bevacizumab.

Treatment and comparator

Intervention: Secondary cytoreduction then chemotherapy.

Comparator: Chemotherapy without surgery.

Endpoints

OS primary.

Key efficacy and clinical outcomes

Median OS 50.6 mo surgery vs 64.7 mo no surgery (HR 1.29; p=.08); PFS 18.9 vs 16.2 mo (HR .82).

Median OS surgery
50.6%
Median OS no surgery
64.7%

Median survival and hazard ratios are not fixed-time event risks; NNT is not inferred from them.

Toxicity and treatment burden

30-day surgical morbidity 9%; postoperative death 0.4%; temporary QoL decline.

Absolute effect and NNT/NNH

Primary OS endpoint negative. DESKTOP III differs in selection criteria; selection for complete resection matters.

NNT/NNH is not calculated unless a directly reported, comparable fixed-time risk difference supports it. Median survival and hazard ratios alone do not provide an NNT.

Critical interpretation and limitations

Primary OS endpoint negative. DESKTOP III differs in selection criteria; selection for complete resection matters.
