Gem-TIP in relapsed germ-cell cancer

Can gemcitabine be added to TIP without reducing TIP dose intensity in first-relapse germ-cell cancer?

Phase I/II mixed-GCT cohort (includes seminoma and NSGCT)

Population
20

Study population as reported in the cited publication; cohort sizes and denominators are stated where available.

Evidence type
Mixed GCT · phase I/II

Not all evidence is randomized, comparative, or seminoma-specific.

Reported outcomes

In the expanded Gem-TIP cohort, the overall complete response rate was 50%, with a further 30% partial response. One-year failure-free survival was 68% and OS 89.5%; toxicity was predominantly hematologic.

Bars visualize source-reported percentages; where the denominator is shown, it is the observed count/denominator rather than a comparative treatment effect. NNT is not calculated for single-arm cohorts or non-randomized comparisons.

Interpretation and limitations

Small non-randomized study; not a standard replacement for TIP and not powered to compare seminoma with NSGCT. The authors called for further evaluation.

Guideline context: ESMO–EURACAN recommends standard-dose VIP/PEI, TIP or VeIP as conventional first salvage options after relapse following cisplatin-based chemotherapy. NCCN Testicular Cancer Guidelines Insights (Version 2.2025) is a contemporary US guideline source. The study summarized here is not necessarily seminoma-specific; histology scope is stated explicitly below.
