EMBRACE II
Can a protocolized, risk-adapted programme of image-guided IMRT, weekly cisplatin and MRI-guided adaptive brachytherapy achieve excellent local, nodal and systemic control while reducing severe morbidity in locally advanced cervical cancer?
Registry: NCT03617133 · One underlying study; aliases and follow-up publications consolidated · Evidence reviewed 3 November 2026.
Study design, population and eligibility
Prospective, multicentre interventional cohort study with protocolized radiotherapy and chemotherapy; not a randomized comparative trial.
The EMBRACE Collaborative Group reported 1,449 accrued patients from 49 centres between 2016 and December 2021. The ESTRO completed-studies catalogue later lists 1,452; secondary reviews report other analysis denominators. This page uses the 1,449 accrual figure from the study-group results presentation and does not mix denominators.
FIGO 2009 stage IB–IVA cervical cancer treated with definitive chemoradiotherapy and image-guided adaptive brachytherapy. The cohort includes risk-adapted nodal field selection and patients with varied tumour and nodal risk. It is not a randomized comparison and outcomes reflect a selected, protocol-accredited international-centre population.
Treatment and comparator
External-beam RT: IMRT/VMAT 45 Gy in 25 fractions with daily image guidance, 5-mm PTV margins and risk-adapted elective nodal volumes. Involved nodes receive simultaneous integrated boost, generally 55–57 Gy.
Concurrent chemotherapy: Weekly cisplatin 40 mg/m², with at least five cycles intended.
Brachytherapy: MRI-guided adaptive brachytherapy with combined intracavitary/interstitial applicator techniques. Combined EBRT+BT planning aims include HR-CTV D90 90–95 Gy EQD2₁₀ (hard lower limit >85 Gy); GTVres D98 >95 Gy (hard >90 Gy).
Key OAR planning aims (EQD2₃): bladder D2cc <80 Gy; rectum <65 Gy; rectovaginal point <65 Gy; sigmoid and bowel D2cc <70 Gy. Hard prescribed-dose limits are generally bladder <90 Gy, rectum/rectovaginal point/sigmoid/bowel <75 Gy. Overall treatment time target ≤50 days.
Endpoints
EMBRACE II is a multi-endpoint benchmarking study, not a conventional trial with a single randomized primary endpoint. It evaluates local, nodal and systemic control; overall, disease-specific and disease-free survival; physician-assessed morbidity; patient-reported outcomes and quality of life; and dose–volume relationships for target and organs at risk.
Key efficacy and clinical outcomes
At the ESTRO 2025 presentation (data cut November 2024), three-year actuarial outcomes were: local control 93%, nodal control 91%, distant control 88%, overall disease control 80%, OS 87%, disease-specific survival 88% and DFS 78%.
For a high-risk subgroup (any N+, T3 or T4), three-year OS was 84%, DSS 86% and DFS 73%. In an INTERLACE-like subgroup without para-aortic nodal metastases, OS was 89%, DSS 91% and DFS 80%.
Crude grade 3–5 morbidity was 8.9% overall: urinary 2.8%, gastrointestinal 3.7%, vaginal 3.2%; fistula 1%. The presentation reported approximately 1% grade 4–5 morbidity in the specified organ categories.
Source-reported outcome visualization
Three-year actuarial disease-control and survival outcomes; crude G3–G5 morbidity is shown separately.
These are single-cohort benchmark outcomes, not randomized treatment effects. The morbidity figure is crude, not a three-year actuarial rate.
Absolute effect and NNT/NNH
There is no randomized control group, so comparative absolute risk reduction, NNT and NNH cannot be calculated. The most informative absolute outcomes are the three-year control/survival rates and the crude grade 3–5 morbidity rate. These must not be interpreted as causal superiority over EMBRACE I or other cohorts.
Trial-specific toxicity, grade 3–5 harms and treatment burden
Crude grade 3–5 morbidity was 8.9%; urinary 2.8%, GI 3.7%, vaginal 3.2%, and fistula approximately 1%. The study-group presentation reported grade 4–5 morbidity around 1% in the specified organ categories. These are cohort-level morbidity figures and should not be relabelled as randomized treatment-related adverse-event rates.
Treatment burden is substantial: 25 EBRT fractions, weekly cisplatin, repeated MRI-based brachytherapy applications, and stringent overall treatment time. The protocol seeks completion within 50 days.
Quality of life
Patient-reported outcomes and QoL were explicit study endpoints. The accessible overall-results presentation provides the principal disease-control and morbidity estimates but does not provide a sufficiently detailed numerical QoL comparison for extraction here. No QoL difference is inferred.Subgroups and applicability
The study reports high-risk (any N+, T3/T4) and para-aortic-node-negative subgroups. These are descriptive prognostic subgroups, not randomized treatment interactions. Risk-adapted elective nodal target selection is a core protocol component.Maturity, limitations and critical interpretation
Design: Prospective interventional cohort, not randomized. Outcomes cannot isolate the causal contribution of IMRT, nodal SIB, cisplatin, MRI-guided brachytherapy or treatment-time reduction individually.
Reporting: The overall results were presented at ESTRO 2025; a full peer-reviewed final outcomes publication was not identified in this audit. Study denominators vary slightly across public sources (1,449, 1,452 and 1,482), reflecting different report/analysis definitions. This page clearly uses the study-group presentation's 1,449 accrual count.
Generalizability: Accreditation, specialist brachytherapy expertise and protocol compliance may limit transferability to centres without comparable resources. QoL detail and full time-to-event confidence intervals were not available in the accessible summary.
Clinical interpretation: EMBRACE II is a major prospective benchmark for modern definitive cervical chemoradiotherapy: risk-adapted IMRT/VMAT, nodal boost, weekly cisplatin and MRI-guided intracavitary/interstitial brachytherapy produced excellent three-year local control and survival with lower reported severe morbidity than historical benchmarks. It supports protocolized high-quality delivery, but it is not a randomized comparison proving that any single component improves outcomes.
