Clinical question

In metastatic hormone-sensitive prostate cancer, does adding docetaxel to ADT improve survival?

Trial context
Population
790 men with metastatic hormone-sensitive prostate cancer
Intervention
ADT + docetaxel vs ADT alone
Key endpoint
Overall survival
Time horizon / follow-up
Median follow-up 53.7 months
Absolute Effect

What changes for patients?

Updated follow-up showed median OS 57.6 vs 47.2 months, a 10.4-month median difference, with HR 0.72. Benefit was greatest in high-volume disease. NNT is not derived from median survival because a reliable fixed-time absolute mortality estimate is not the appropriate basis here.

NNT: Not appropriately calculable from median survival

Survival outcome — visual comparison

Median survival is shown as reported; the 10.4-month difference is not a fixed-time absolute risk reduction and should not be converted into an NNT.

Treatment burden / harm

Docetaxel treatment toxicity

NNH: Not quantifiable from the selected primary endpoint; febrile neutropenia and other chemotherapy toxicity reported in text

Relative effect

HR: 0.72

95% CI 0.59–0.89

Clinical interpretation

Docetaxel added to ADT improved survival most clearly in high-volume mHSPC; the absolute value depends on disease burden, fitness and the modern choice of ARPI doublets or triplets.

Current-practice context · reviewed 14 October 2026

CHAARTED established an overall-survival benefit from adding docetaxel to ADT in metastatic hormone-sensitive prostate cancer, most clearly in high-volume disease. Current care generally intensifies ADT with an androgen-receptor pathway inhibitor, docetaxel-based triplet or another guideline-supported regimen in suitable patients; docetaxel alone should not be assumed to be the preferred intensification for every patient in the modern era.
